She could get a bit of mileage out of going down the Runnersworld route.. health magazine interviews and tips, invites to be at local races, etc. The non-elite who beat elites in Americans most famous race can inspire in local running communities. She could do at least a good year of that.

Yeah and the other magazines will follow suit. Women magazines, medical magazines etc. Good example was a 42-years old nurse who won the marathon in the Finnish championships last year. She has been probably in every magazine after that and received a sponsored Audi etc. Stories sell and when a career woman or mother does well, it is even more interesting.
Certified nurse anesthetists make $150 -200k / year. No way she'd get close to that as a runner [even as a high 2:20s type]. She'll keep working and running on the side for sure.
I think it's a great "feel-good story". Woman works full time job as a nurse, trains on her off-hours. Finishes 2nd in the Boston Marathon. No sponsors. No agent. Self-coached (from what I can tell).
If anything, she is more of a "blue collar runner" than Des Linden is because of the above.
Can she capitalize on this? Sure. She's more relate-able to the average runner, I think, than a Shalane, a Des or a Hasay. Sponsors love this kind of story, but she needs to strike while the iron is still really hot. It's not a very big window at all and to be honest, she'll probably be gone from the public's eye in probably a week (if that).
That being said, I'm not sure I'd quit my day job. It's a great job, making a difference in peoples' lives and pays really, really well.

Google tells me the avg salary for nurse anesthetists in Tucson is 160k. She's new in the field, so probably doesn't quite make that.
But still, there are so many levels of sponsorship she could get. I could see Oiselle or New Balance (they have like zero marathoners right now, no?) giving her shoes, entry fees, travel, and maybe a training stipend for massage/PT/stuff like that. Seems like it would be a mutually beneficial relationship and worth it for both sides.
Wait, it takes 7-8 years of practical experience for a nurse to gain CRNA status. Sara is only 26. She must have been a childhood prodigy. Regularly performing procedures at 12 years old, damn!

Her husband is a Orthopedic Surgery resident. I would think he could support her now or eventually while she runs? (Sidenote, that's so "Grey's Anatomy" that they met in the operating room-she mentioned that in one of her interviews) I agree that she should try and capitalize on everything now. Hopefully she doesn't hook up with Oiselle, though. Does she have a social media presence because I can't seem to find it. Pretty cool that she runs before or after work at 4am or 7pm. The working person loves "relatable" stuff like that. Already has a solid coach, just needs to stay healthy. I'd think she'd be able to nab the OTQ at her next marathon. Would be cool to see her at the Marathon Trials in 2020.

Depends on how outlandish their student loan payments are, a surgeon and nurse anesthetist, she already said the 75K was to put a dent in their loans, a dent, but she really could profit from this if she works quickly like others have said above

And she showed that she is way more tough than those who think they can move on to the next race. She'll likely run it again next year, and maybe run a similar time. A bit better if the conditions are better. She ran it, she didn't race it. Didn't even know where she was in the field until they told her she'd come second.
A lesson wannabees should take is that working and competing are not mutually exclusive. Often work is a grind, and you have to grind it out. On a crap day, she still has to work in the Operating Room. She has an important job. Finishing a run in crap weather is only important to hobbiests.
Great feel good story but it was a perfect storm (pun intended) of circumstances leading to her second place finish. Not repeatable.
