In my day job I'm an historian, and I think it is interesting that there is not a good intellectual history of the development of the science of distance running and coaching. Or perhaps I've missed it. Can anyone suggest something?<
One section of Tim Noakes' "Lore of Running" takes a look at this, although not quite in the depth you might like. Other places in the book specifically address how scientists developed, used, and possibly misused, VO2max. A similar look at altitude training is also in the book.
