Why is it degenerating?

With an arthritic foot, your running days should be over. This isn't something you want to mess with.
There is only one treatment that can regenerate diseased articular cartilage. Average doctors, including specialists, aren't educated about it and can't provide it.
I would look for more opinions. Try to find someone who understands runners. I had a doctor tell me I was done because of neuropathy (originally went in with pain in my left foot). The diagnosis was correct, but the doctor didn't understand there are different extents of what I have. Went to another doctor after driving myself crazy from not being able to run after a few months. A few years later, and I'm running more than ever.
I also know someone who was told they have arthritis in their foot when they graduated from college. Almost 15 years later, he is still doing 120+ mpw.
So, definitely get another opinion.
Had a stress fracture of the navicular and the surgeon put in screws that were too long...
