I don’t want to name names because of confidentiality concerns, but I am aware of an elite athlete from my local university failing a random drug test a few weeks before Indoor nationals and being left home- the team lost the national championship by about the number of points he would have scored.
This may or may not apply but I've heard on several different occasions that major NCAA football teams are informed ahead of time when their drug tests will be so everybody shows up clean.
When I was on college they tested at nationals. The winner plus randoms after the races. I got pulled into one of the randoms. I remember being really mad about because I ran like crap and just wanted to get out of there.
They have institutional tests, conference tests, and NCAA tests.
For my school, the institutional tests you get notified the day of and come in and do the usual routine. Everyone on the team does it. If you fail, ie probably marijuana or steroids nothing will really happen. Marijuana=slap onthe wrist. Steroids=coaches are probably going to sit you a few meets and ensure you stop taking whatever sketchy supplement you are on.
Conference tests they grab "random" people. This will be the better people on your team and not the slobs. Last time I got tested I was sure I was going to fail and never heard anything about it so...
NCAA tests presumably same as conference tests. Both are going to get you suspended for a season for a positive test. You aren't going to hear anything about it since the NCAA and team aren't going to publicize it. They'll just claim the person wanted to redshirt.
I should add that some people on team somehow always knows ahead of time when we are having a test and let the team know the day before.
