I had surgery when I was a teen and it stopped it from hurting but didn't change the physicality of it really at all. I understand the technology has improved in the time since then. I'm your age.
I went back in when I was moved off of my parents insurance and the trainee first looked at it then went out to consult with the boss His boss or whatever the technical term is came in and said, 'oh that's not that big, .... well not the biggest I've ever seen at least' so .... cool I guess.
Anyway, his advice since I had no pain was to do literally nothing. Even if I were trying to have a kid, he said first give it a shot as is and then if nothing's working you can think about surgery. I never had a semen analysis done but based on the fact that it's apparently off the charts of what the new guy had seen I'd assume things aren't great.
