If someone who runs 12 min miles is training for a marathon, and if the general rule is no continuous run of more than 3h, how do people running that pace ever get to 20-22 miles in training?

They really don't need to. They are training to do a 5 hour event. This is comparable to top runners training for a 50 mile race. In general, those people don't do 4 hour runs (except for the Japanese, who will do them for marathon training).
