The treadmills at my gym have a built-in 1 degree incline (they say zero degrees but they're actually uphill by 1 degree). So that's one issue. Another is that the air doesn't circulate as well when you're indoors, so I overheat more quickly and it's harder for me to breathe. The surface also makes a difference, the way some treadmills bounce sort of absorbs the energy from your step and makes it a bit harder for you to push off.
