If you're referring to Lagat, I think the problem many have is that he's likely still benefiting from the EPO he took that enabled him to run 3:26 all those years ago.
Up till now I thought it was accepted as common knowledge that all east African runners are at least 5-10 years older than they claim.

They are expensive but IAAF has money. Also they can just limit the tests to those who end up on the podium.
It does seem to generally overestimate ages by around 5-10 years. So really we could say that it thinks George is 21 and Jakob 17, and the Ethiopian girl 41.

I uploaded a bunch of pictures from a recent photo shoot to test that AI. It is a 35 year old trainer and a 33 year old model, I was the photographer for the trainer's website. For the trainer, 5 pictures gave a span of ages from 16 (wtf, she doesn't look 16 by any stretch) to 40. For the model, it gave a range from 13 to 34.
I don't think that is any sort of authority or proof of age cheating...
I went to college with a girl who was 18 as a freshman, but looked 40 at first glance. These people exist. Is this woman cheating? Maybe. But there is no proof or even "certainty," only suspicion.
