Im a ruppster for life but this seriously makes me question his intelligence.
Look at the back of his hand on his wifes back upside down.

Funny never noticed that. This picture was from his first marathon. Most likely Alberto drummed into his head how much he had to drink. That was a hot day. Pretty easy to get caught up in the excitement of your first marathon and skip a few drinks. If you have issues with the heat you can get pretty loopy. Been there.
Maybe the other hand says "Run"
I think that’s fairly normal. I’ve written messages on my hand in sharpie before a race as well.

Was this that time he did 2:09?
The message is not reminding him to drink. There is an arrow on his hand to remind him which side of the street the personalized drink tables are on. On a wide street, you don't want to have to swerve across the whole street to get to your bottle.
Did he have a fanny pack to store his sharpies?

It's to remind him which side of the road his drink is at, not when to drink...obviously it's every 5k
Ban him for life. That's an unfair advantage.

But when he is running that arrow would be pointing up the road, in the direction he is running.
Sometimes I have to remind my more obsessive athletes to take a rest day. Don't like the anti Rupp stuff here.
And even it was a reminder to drink, many runners and non-runners have for years been using devices to remind them, so nothing new here. What would be cooler is if he had an electical impedence sensor that detected incipient dehydration, and yelled at him to drink. Technology is not new, it's basically the same doohicky on some bodyfat scales.

I sometimes write my track events on my hand, if I’m doing something different than my normal lineup.
