Mo Farah was given the intravenous prescription drug/supplement before the 2014 London Marathon, in which he finished EIGHTH PLACE!
The doctor who gave double Olympic champion Mo Farah a controversial supplement has told MPs that he failed to properly document the treatment.
Dr Robin Chakraverty said the amount of L-carnitine was well within World Anti-Doping Agency (Wada) limits.
Dr Chakraverty said the substance was given to help the Briton's performance.
He said he gave 13.5 millilitres of the legal supplement, below the maximum allowed of 50ml within six hours, by injection and not via drip.
Ed Warner, the UK Athletics (UKA) chairman, told MPs that not recording injections was "inexcusable" although Dr Chakraverty said record keeping had since improved.
The use of the substance, given to Farah in 2014, is being looked at by the US Anti-Doping Agency (Usada) - which has called it an infusion - to determine whether rules were broken.
The injection was made in consultation with Farah's American coach Alberto Salazar.
Warner said it was "disappointing" the injection had not been recorded at the time by the doctor.
At a meeting of the committee earlier this year, a doctor who received a 'mystery package' for cyclist Sir Bradley Wiggins in 2011 said he had no record of his medical treatment at the time.
Ex-Team Sky medic Dr Richard Freeman had a laptop containing medical records stolen in 2014, the committee was told in March.
Team Sky and British Cycling's record-keeping was questioned at the earlier hearing and when it was raised again on Wednesday, Warner replied: "Please don't tar us with the same brush".
He said UKA was keen to centralise its records and now handles all of Farah's medical care.
"There was a period of a few months in which we allowed Mo to go to Oregon and be treated by a local GP over in America, and we were observing his medical care from afar," said Warner.

Yes a good matchup but seems like we get something similar about every year but then doesn't live up to the hype. Does anyone think Kipchoge will not be the heavy favorite?
What do you think the optimal distance would be that would give each of them an equal shot? One hour run? 20 miles? I think either of those would be more compelling.

Instead, Bekele should stop running completely, balloon up to 200 lbs, and still beat Nick Symmonds in the Eugene marathon.
Are the Tokyo fields out yet? That could pull a few big guns away from London? It's sort of silly that we have the full Boston field, some names for London, but nothing on Tokyo which is a month from now.
I wish Bekele wins this race with a WR and Kipchoge have it first marathon loss . Remember how shocking it was when Bolt and Farah lost in WC london

It's just in the last few years Kipchoge and Bekele have decided to forego it only because they feel they need to own a WR. can't do that in Boston. I won't respect Bekele or Kipchoge as much until they win a Boston. That's that.
" All the top guys have always run Boston" Uh? Not recently. Boston is a good race for a guy like Rupp, just as it was for Alberto in early 1980's (also New York). Coach Alberto, undefeated in the marathon until he went to Europe to get creamed. At least Galen has a bronze in the Olympic Marathon, I give him that and respect him for that.
These days kenenisa is a less factor. kipchge is the real deal and he be the third person to win olympic successfully in tokyo
If Farah gets the British record (i.e. runs 2:07 or so) it will be presented as a massive success. Sub 2:06 would be huge. I don't think many people expect a win - Kipchoge is known to be a big deal even among casual fans in the UK.
