One of the theories that my docs are pursuing is that I might have an auto-immune clotting disorder. If so, could be genetic or acquired, but either way treatment would be anti-clotting meds rest of my life. Failing that, they really have no explanation.

I don't mean to diminish your issues, but it's quite possible you have excellent cardiac function and your arteries overall are clear, and that the clot was a one-off that caused the problem (or it's a clotting issue in general as you mention).
Heart function tests can be done to measure ejection-fraction, how much blood leaves the heart each time it's pumped. This is done with 3d ultrasound. I've had it done, and talked to the tech to allay my anxiety during the test - he said he could not give results (the Dr did later of course) but he said it's very obvious when people have poor values for this test...and i shouldn't loose sleep over it (Dr confirmed, all good).
Another test is endothelial function and measuring intima thickness, also via ultrasound (other tests as well). Higher values for intima thickness are indicative of atherosclerosis, as well as the problems of having narrow arteries (higher BP, less flexible etc). Many sites will explain this better, here's one link that talks about it in light of cholesterol:

p.s. why did dr order 3-d ultrasound for my heart, despite running for 20+ years and being only in late 40s? I had skipped heartbeats, odd ekg ,and enlarged heart. The enlargement was right ventricle from aerobic training and overall not a concern because the heart chamber had also grown in proportion. Some people get enlarged hearts but chamber is small, which results in low ejection-fraction and is bad news. My only issues are (benign) skipped heartbeats, the Dr says the ventricle is so long, and my HR so low, that minor re-polarization issues manifest with occasional skipped beats.
