Is a prescription required for said substance to be used for medical purposes? Was a therapeutic exception filed? What medical route was used for its procurement?

If it's not banned, no TUE is required.

That was my reaction as well. I wouldn't be surprised if very legitimate runners took Meldonium up until this year - especially those who like to get to the bleeding edge (like the NOP). Then again, just because anabolic steroids were finally banned by the IOC in 1975, does not mean it legitimizes enhanced performances before then. It looks like meldonium is in the same category as AICAR and insulins, so it looks pretty serious. I don't know anything about it besides that, though.

If any state running a doping program just needs to approve a PED for therapeutic use in order for it to be allowable in the (many) years before it is banned, it shows what a exercise in futility drug testing is. Clearly she wasn't suffering from heart problems, which is what the drug was approved to treat, I believe. And it took something like 10 years after its initial development before it was banned.
At the very least, athletes should have to declare the drugs that they are taking, demonstrate medical necessity, and agree that if a drug that they are taking is subsequently banned that they will retroactively forfeit medals and have records expunged. Perhaps that would result in an occasional injustice -- but the toothlessness of the current system is absurd.

You do realise that this is the only thing she's been caught taking, the chances that's it's the only thing she's on or has been on in the past is a bit naïve.
If she's prepared to take a pill meant for heart conditions, legal in sport or not, it's not a stretch to believe she's probably popping whetever she and her people can get her hands on.
Have a think about what you'd do if you were offered a random drug to take to improve your performance, even if it wasn't banned at the time, would you actually still do it? I know I wouldn't be knocking back random, Eastern European heart pills.
Interestingly, according to the interview with her agent (link on the LR.com home page), it sounds like she is totally denying taking the drug. She has "no idea how it got in her body". She doesn't explicitly say she has never used meldonium, but that clearly seems to be the implication from what her agent said. (if she had taken this drug in the past, but quit when it was outlawed, she would presumably say something about how there must have still been some in her bloodstream that triggered the positive)
Also, gotta love the "she looked me in the eyes" defense. She's being honest, people - SHE LOOKED HER AGENT IN THE EYES while denying drug use! Everybody knows you can't simultaneously lie and look another person in the eyes. That's how Phil Liggett knew that Lance Armstrong was telling the truth about not using PEDs - he used the old "look 'em in the eye" test. 100% reliable.
This latest story makes me even more suspicious of Aregawi.
If she said she had been taking meldonium when it was legal and claimed that she didn't get the memo that the rules had changed, I could sort of understand that. But to claim that meldonium magically got into her bloodstream makes me wonder what else she is lying about.
