Good question. I do not look back. I listen carefully, and try to be aware of footsteps. The last time I was in a race and someone got close, I heard the footsteps, and then, according to what she said after the race, I "took off." I was surprised she said this, as I kind of zoned out, focusing on my pace, and did not quite realize that her footsteps had faded. It was in the last mile of a 5k. Be confident, and attentive, and do not worry. The better runner will win anyway.
You should run as hard as you f*cking can. The only reason anyone looks back is to judge whether it's safe to coast. It's a display of fear.
Of course it is bad form to look back, you know that and so does anyone that knows anything. Looking backwards whilst running is not an efficient running style.
However it is often a necessary tactic in heats and semi-finals, to ensure you can safely relax as you approach the line. Even sprinters do this.

I try to be aware of where other runners are, but I did almost get caught at the end of an 800M earlier this year. Beat her by a smidgen. Never heard her coming, but I was running all out the entire time, so no regrets.
Usually I am the one chasing other people, so it's not an issue.
