Believe me...I am no wimp. But, I do have anger management problems that require me to avoid confrontations.
I use to drive a cab in the City. I was 'the captain of my ship' and didn't take shit from anyone. I put drunks, sitting in the front seat, into the windshield. I tossed women out of the cab in undesirable places of town. I did some pretty unacceptable things when people pissed me off.
I did things I'm ashamed of now, but I have matured.
But even then (30 years ago) my ego wasn't so fragile that having some old broad telling me to put my shirt on would have gotten to me.

They probably give you a hard time because they know you're a yankee. I've never had any problems running shirtless, and I'm in the heart of the "bible belt". Only people who've told me to put a shirt on were some 6th grade boys making fun of my shorts as I did some intervals around the track. Then one challenged me to a race and embarrassed himself in front of his little buddies.
As far as saying "no" vs ignoring her, ignoring would probably be more effective, as I doubt she'd continue to ask you to put one on, but "no" would probably p^ss her off more, and lead to her trying to argue with you or something.

You put your shirt back on??? She was WALKING laps. I run shirtless in Dallas for much of three seasons. I'm 5' 8" and 145. I love it when Dallas women run in buns and sports bras. Your assailant is a repressed Puritan who needs the First Amendment explained to her. Also, walking of the kind she was probably doing isn't exercise. Perhaps NC is more conservative than Texas, which is hard to believe.
