For what it's worth, my copy -- a first edition from 1982 -- indicates that the publisher was Doubleday & Company, Inc., Garden City, New York. If the publisher can't help you, perhaps Moore himself can. (For some reason, I thought that he was living in Hawaii these days while working on a book about Bowerman, but I'm not sure about that.)
Last word on Moore that I got is that he's temporarily back in Eugene while working on the Bowerman book. That was maybe a half year ago and I THINK I recall hearing that he'd be there for a year or so.
I actually just picked-up a copy online for $35 (hardback first edition) for a friend for Christmas. Just check the used book sites online from time-to-time and you will find one eventually. Amazon is not the best place to go. Try

There is a copy in my local library which has not been checked out since 1987. I tried to convince the librarian to sell me the book (the lost book price was $8) but they would not. I am tempted to just check it out and then say I lost it and pay the fine.
This doesn't help answer the original question, but Kenny Moore has to be one of the two or three best track/running writers.....maybe THE best. Profiles of track & field athletes that he wrote for Sports Illustrated were always top notch.
It doesn't help with the original question unless you wanted to track KM down. But the big Kenny Moore question is what he does for money these days. Its been years and years since he's written anything for Sports Illustrated, or for anyone else as far as I can tell. The royalties from "Best Efforts" and "Without Limits" can't be that big. And he hasn't gotten an advance on the Bowerman book.
Thanks for the help with the used book links. The only thing I've come up is first editions going for $125. Anyway, regarding what Kenny Moore is up to, I knew that he was splitting time between Eugene and Hawaii, and the only thing I've seen in SI is his profile on Mamo Wolde done in 1995 chronicling Wolde's prison stay in Ethiopia. Anyway, there's a pretty good interview done in August with him in the Portland, ME paper.
