I have insertional Achilles tendonitis and I seem unable to find a PT who follows any of the direction that seems to be commonplace when reading online. Everything I've read from the studies to testimonials, to write-ups from experienced PT's all seem to suggest:
• No stretching,
• Body weight eccentric heel drops
• Gradually increasing load with eccentric heel drops until strength has returned enough to allow for a return to sport.
• Reincorporate stretching before intense running or exercise
After 9 weeks, I'm on my 3rd Podiatrist and 3rd PT and 6 days of prednisone. All 6 of them have to insisted on the first phase of recovery being intense stretching routines. Every time after a few days of stretching the pain has gotten far far worse and then takes a week or 2 for it to settle down. The current PT is highly recommended by the running community and works with a lot of marathoners and has me stretching out using a strap laying on my back instead of under load. It does not seem to be re-injuring but after 8 days of stretching... I can't tell that anything is improving.
What I can't figure out is why every single PT and MD insists on stretching the achilles while all the information and studies I've read have said DO NOT STRETCH for insertional achilles tendonitis. I'm going on 2 month with this and still can't walk more than about 1,000 steps a day without the pain returning by the end of the day. I'm not trying to get back to playing sports - Just the ability to walk around my house and the office.
I'm 35. In my life I've had 2 torn rotator cuffs which I was told required surgery but was able to fully rehab with PT back to 100% functionality playing college Tennis and and had
4 lisfrank partial tears in my feet, again all fully rehabbed without surgery. I'm very familiar with PT to get over injuries but I've never experienced anything like this. With all my other injuries with plenty of rest and following the PT recommendations each day is slightly improved, but with this injury I'm 9 weeks in and have zero improvement from when this originally started.
Are these PT's just uninformed or is all the information i've read simply incorrect? How does one actually find a PT that knows what they are doing?
