OK conceivably if you've been starving yourself of carbohydrates and had a one-in-a-bajillion adult-onset intermittent MSUD, this scenario could be possible. But it's many orders of magnitude more likely that this guy is just pulling cool symptoms off wikipedia. Next thing you know his sclera will turn blue.

Agree, this person pulled this straight off of wikipedia. Maybe next, I have these blue, "Koplick spots" in my mouth, what could it be? Who would actually describe their breath as "fruity acetone." That's straight out of a textbook and if they actually were in DKA they would not be wondering casually what it is as they would be sick sick, like go to ER I'm in trouble.
