Like some people have said, it would need to be a treadmill game. Still, some interesting possibilities.
You could line up a whole series of treadmills in one room, each one with a screen showing your position on a track. You could set in a handicap, so an 18:30 guy gets a min/mile handicap against a 15:30 guy, so teams or other groups of people of different abilities could race each other in real time. You could have a wireless baton or something that allows you to move left or right and speed up the mill or slow it down by flicking your wrist.
You could apply the handicap to some sort of "legendary races" option, whereby the computer opponents represent real racers in real races. Imagine an 18:30 guy giving himself a 1:45/mile handicap and racing the 1976 5K. -
