Like some people have said, it would need to be a treadmill game. Still, some interesting possibilities.
You could line up a whole series of treadmills in one room, each one with a screen showing your position on a track. You could set in a handicap, so an 18:30 guy gets a min/mile handicap against a 15:30 guy, so teams or other groups of people of different abilities could race each other in real time. You could have a wireless baton or something that allows you to move left or right and speed up the mill or slow it down by flicking your wrist.
You could apply the handicap to some sort of "legendary races" option, whereby the computer opponents represent real racers in real races. Imagine an 18:30 guy giving himself a 1:45/mile handicap and racing the 1976 5K.

Not everything that is fun to do makes a good video game. Luckily, running doesn't have to be made into a video game because it's something that you can do in real life any time you want.
You can also go fishing, skateboarding, have your own paper route, steal a car and kill someone in real life, but all things have all been made into video games as well.

It's a great idea. You control your workouts. Have to decide between building a long term base or getting glory while you're younger.
Then for race day, decide pre-race meals, fueling strategy, pace to go out at, when to push it, surge, make your decisive break, when to kick for home, etc. Also your race would affect your recovery time for your next race. You'd have to decide what meets to run at balancing pschological and financial aspects. Also do you risk career health issues like Salozar in the race in the sun where he almost killed himself and never recovered. Yes EPO and other cheat options with risk of lifetime ban. I'd love to play.
