TONS of stretching the IT band, quad, hip flexor and hamstring. Also getting active release therapy done on the IT band, hip flexor and quad. In addition, doing strengthening work on the quads and hips (squats, lunges, etc). The stretching will help get your tracking back in place and the strength work will help you hold that tracking in the right place.
It's happened to me, and I have some interesting x-rays that show it. It was because of scar tissue just outside of my patellas. They were preventing my IT band from letting go of the patella. In addition to the stretching and physical therapy, my PT had me doing VMO exercises. That's because the vmo becomes less used and thus weaker when you have this condition. I squeezed a pillow between my legs with my legs straight, and slowly bent my knees and back up. Squeeze extra hard on the way up.
Obviously you need a good PT who can put together a treatment plan. Watch out, there are lots of hacks who will try to sell you orthotics and get you to have surgery done on your knees. Write again if you need a good ref. in the northeast.
Strengthening of your quad muscles and your hip abductors are 2 important parts of the treatment along with stretching.
Do not completely discount the fact that orthotic devices can sometimes help with this. Go the rehab route first, if you are still experiencing pain get someone to check out your biomechanics.

"Excessive hindfoot pronation results in prolonged internal tibial rotation during gait, adversely affecting patellar mechanics. Orthoses can sometimes help control overpronation, though they are not routinely prescribed"
