No, you will get a cardio and lung efficiency improvement. Even a total old couch potato will get this.
Years ago I had to make weight for an insurance physical I was out of the BMI range and hadn't run as a casual jogger in 20 years and as a school boy runner 30 years earlier and I had all of these improvements within a week and a half.
I wouldn’t worry to much about running fast, your symptoms sound exactly like heart cancer.
What are you using to measure improvement?
Do a mile time trial. If it doesn't get better by one second+ then the runner has probably injured themselves.

yes it is possible if you are doing the wrong type of training.
It is possible. I did it. If you are young then it is not very likely. Bit, if you are middle aged and in bad health it can happen. I was 38, sleeping 3-4 hrs a night, drinking every day, living on junk food and wondered why I didn't get any benefit when I started running. Eventually, I tried being healthy and went from 25 min 5k to a 20 min 5k in 6 months.

Training inconsistency sounds like what is holding you back. By running "on and off", what do you mean? What was the longest stretch you have been consistently running for? When you were "off" of running, were you sedentary? Being inactive for an extended period of time is definitely going to kill the fitness gains you made.

Probably 21 for women, 16:30 for men.
Of course I was sedentary and off for months. I can't workout when I am injured and in pain.

You are probably tired.
It take a few years to get into shape from nothing.
