This wasn't bright on her part but I also think the officials should have informed her to get in the right place. They can get into the exchange zone right after the start and every meet I ran had officials corralling the athletes to be in the right exchange zone.

Assuming the US athlete wasn't misled by an Olympic official, I'm blaming the Team USA coaches here.
These are athletes who want to focus on running as well as they can at the biggest meet of their lives. The stress/nerves before the event cause them to naturally lose focus on the little things like where they are standing for handoffs, because they want to focus on their actual run. It's up to the coaches to tell them where to stand and to remind them if they're having a brain fart.
Coaches have held their hands throughout high school and college, so they need to do the same thing at this level, "just in case."

There is only one exchange zone like this on a 4x4, so she maybe never runs 2nd leg. However, there had to be practice with the proposed relay lineup and that exchange zone should have been drilled into her head.
Another thing is that track and field needs to administer time penalties rather than give straight DQs for minor infractions. Exchange outside the relay zone? Add a second to the time. Step on the line in the 100m or hurdles? Add a tenth of a 2nd. Etc etc. yet another example of track and field cannibalizing itself over stupid, non-consequential rules.

This falls on Lynna Irby. She has been in tons of 4x4s in High School and at the Univ. of Georgia. She knows where the 2nd leg runner lines up at, regardless if she has never run 2nd leg. She could clearly see that she wasn't lined up correctly. Did she actually think the other7 runners were in the wrong place? In the video you can clearly see the official yelling and pointing at her that she is not lined up in the zone.
In the 4x4 you have to be lined up in the 20m zone and the baton must be passed within the zone. The rule is fine, suggesting to add a time penalty is ridiculous.
Watch the race video and interview with Irby afterwards by Lewis Johnson. Irby was clueless. WTF?

my guess is officials placed them in the wrong zone. looked like a number of teams were scrambling back to the exchange zones at the last second. wouldn't be the first time officials have messed up. remember that steeplechase? or the shanghai 800?
