He passed protocols to travel to Tokyo (testing in the US) and to enter the Olympic Village (testing in Tokyo). You then are subjected to daily testing in the Village, where he tested positive in at least three subsequent tests.
The most likely scenario is that he was negative going into Tokyo, caught it either en-route or in the Village and is now positive.
It’s possible that Sam did nothing “wrong” per-se. These are just the risks of carrying on with the Games under these circumstances.
If I were advising athletes in the Village, I’d recommend vaccination and diligent masking to protect your status and that of your teammates.

That’s the main goal of the vaccine.
And it reduces transmission rates.
You may not get sick, but you still can contract it.
That’s true for vaxxed or unvaxxed.
So, if you want to compete in the Olympics, you may want to take extra precautions of distancing and masking to reduce you risk of a positive test.
(Don’t high five the whole crowd at a meet before getting on the plane, for example)
I think I would be extra precautious in the weeks before heading over.
Make it it to the day of your event at least.
Vaxxed or not.
This is in the same lines as don’t get into a pickup basketball or soccer game.
Avoid chances at injury.
You also have to wonder at a guy who didn't do everything possible to keep himself healthy and/or able to compete in the Olympics, knowing the rules. Don't blame the system.

One unvaccinated U.S. athlete tests positive. He’s interacted with dozens of other American athletes, presumably most of whom are vaccinated, and they have not (at least yet) been forced to withdraw.
Imagine thinking these facts make the vaccines look bad. Kudos to the American athletes who took a very simple step to protect themselves and others from heartbreak in Tokyo.

Not one positive test but three consecutive ones. And he hasn't tested negative yet so that's not a "sandwich", unless you make your sandwiches with just one slice of bread.
This is pretty much how testing works. You get lots of negative tests, until one day you get the disease and test positive. Incubation period is 2-14 days so he probably contracted this back in Covid- infested Mississippi.

Spot-On. You can be anti-vaccine or just not want to get it for a number of reasons. Unless you have a very recent infection (a la Cole Hocker) conferring some protection it is taking a big risk to not get it when you will be tested daily. This isn't even a health/ethics issue, it's a practical one that professional athletes face.
My kids will be devastated that they won't see him. Real shame!

Gutting for Kendricks. By far my favourite American athlete. Truly world class as an athlete and a human being with the way he competes with a smile on his face and wholeheartedly cheers for his competitors even in the heat of battle for global titles.
The Duplantis-Kendricks duel was one of my potential highlights of Tokyo.
If he was vaccinated why not just say you're vaccinated?
