Bannister got under 4:00 on maybe 25 a week. Bowerman had a guy who probably did the same. When Bowerman would send them out for a distance run this guy would run until he was out of Bowerman's sight and sit down somewhere to wait for the rest of the team to get back. He'd go to a restroom and dump water over himself so it would look like he'd been running. But for most people there is neither a minimum nor a maximum.
You need a combination of speed, stamina and durability to accomplish any distance running goal.
The specifics vary both from runner to runner and from event to event.
For 4:02 -4:07 1600m you typically need to be able to split a 51-52 on the 4x400m, running start of course, nothing from blocks.
The minimum mileage requirement here ranges between ~15 and 45/week depending on the athlete; no one size fits all.
If on the low side of this mileage range some additional cross training and strength training is generally mandatory, but none of that supplementary activity requires upper body work, aside from running in the pool.
I never ran a mile in that range until I hit above 100 miles in a week. But I was a 10k guy running high 28's for 10k, and the mile wasn't my specialty.
Depends on the athlete. An enormous talent who benefits greatly from pure speed work could do it on 20-30 mpw. Most athlete I know in that range are running at least 60-70, many getting as high as 100+
Depends how you respond and where you are now. Training smart always beats training hard, but most people don't know how to train smart so they hammer themselves to train hard. For most, a 4:XX mile is basically a VO2 max test with anaerobic and aerobic energy production required and the ability to produce, process and use lactate efficiently. Both your aerobic system and anaerobic glycolysis systems will be firing.
The minimum? Somebody like Webb or Brazier could run 4:07 on 15 MPW.
Way less than 50 for many runners.
