I am in my early 20s. Ran track/XC in college. In May/June I was really hammering my training and increasing volume week after week from basically nothing in COVID to 80mpw. In mid-June I developed a sharp pain on the bone at the back of my left foot and completely stopped running. I can walk and move the foot just fine, but I can't jump let alone run for a period of time without pain.
I spent 6 weeks doing PT (eccentric calf raises, stretching, massage, hip strengthening, etc) with no improvement. I got an ultrasound in August confirming a partial tear of the tendon at the insertion.
In early September I did a few rounds of ESWT to try to jumpstart the healing process. I followed this up with 3 weeks of PT similar to what I did in the summer. No improvement.
At the end of October I got a PRP injection, followed by 2 weeks in a boot, and am now in my 3rd week of PT. No improvement.
I still have a very sharp-to-the-touch pain at the back of my heel, and can't load it heavily. I can bike and surf without much pain as those are pretty low-impact, but still can't run.
At this point, I feel stuck and am getting all kinds of different opinions from various doctor's, PTs, etc. Some suggest totally immobilizing to "let it heal" for 6 weeks, but my activity has been pretty low for the last 6 months and rest hasn't helped in any way. Some suggest starting a progressive-loading protocol in which I do progressively harder calf-raises, then little hops, then light running. But I've gone pretty hard on PT in my last few attempts and seen no results. The nuclear option is surgery, but it's not recommended to operate on a mostly healthy tendon, and it's not clear what a surgeon could actually do.
I'm stuck. Nothing seems to help. Some say rest, some say work out, some say get surgery. What do I do? Has anyone had any experience with this?
