This is not a First Amendment issue. This is a question of personal choice and personal responsibility. If you believe in the Bill of Rights, you should also believe in the concept of personal responsibility. In this context, personal responsibility means wearing a mask and taking the year off from family gatherings that will spread COVID.
Some would say it's a matter of personal responsibility to ensure your older relatives are taken care of emotionally. But, sure, the only thing that matters is they don't get the covid, even if that means extreme loneliness and anxiety.
Hopefully your elderly loved ones aren't alone and are doing just fine. If so, great, stay apart. But don't put some one-size-fits-all hot take out there and expect that it's the only perspective.
