hip impingement/ labral tear surgery

Same issues here justin...sports hernia for a while then hip issues. I can bike a bit but walking around is painful and the glutes do not seem to fire at all during running. Even standing and shifting weight from one foot to the other is painful. I've had the sports hernia issue for about 6 months(but it is getting better, or maybe I just notice it less because of the hip) and the hip issue for about 3-4+ weeks...wish I had seen this thread before today!

i am about to schedule my fai/tear surgery on one side...ouch!

i agree that fai precedes sports hernias. no doubt.

sports hernias are way behind me but the hips have become more prominent.

just saw a dr in nyc regarding resurfacing but my arthritis is basically nil.

Had bi-lateral sports hernia repair two years ago and no pain. Still can't run because of torn labrum in hip and FAI. I am doing one side in November. Not sure if Ill have to do the other side but I think probably so. What do you think came first?

i think the fai came first. the torque of the hip dysplasia causes the soft tissue to become hypertonic, thus it eventually rips. but the bony issue, the cause, still remains.

i am trying to get my surgeon lined up. either kelly in nyc or phillipon in vail. i'd rather go to phillipon but logistics are rough with flying and all.

I've been diagnosed with FAI and labral tears. I've been referred to Dr. Stubbs at Wake Forest University Baptist Hospital for surgery, but I have not consulted with him yet.

I understand the the rehab can be quite lengthy. For those who have had the surgery to repair the FAI and labral tears, how long were you out of work? I'm an accountant and I SIT at my desk all day.

I know what you mean. I had a groin/hip injury in the fall and I am still recovering. I had ART treatment for a while and a lot of my thigh, it, and hip pain is gone. However, lately it feels like my inguinal ligament is becoming inflamed and my lymph nodes are enlarged. My iliopsoas feels strained too. Any suggestions? ice? stretch? I am only doing 40-50 mpw when i am normally at 70.

any help would be great

My understanding of this is that its a postural/mechanical issue.

What worked for me is to keep the knees slightly bent at all times, with body weight over the ball of foot. Keeping the upper body erect, u will find that there's a slight forward lean fr the ankles, not fr the waist. This puts the glutes in a fully functional position and lessens the strain on the hip flexors and knees. U should also find that yr transverse abdominus (TVA)and entire core area activated automatically to fortify yr trunk.

But this means that developing solid Core (esp the TVA) and gluteal (esp gluteus medius) strength.

I tried doing some core work, but I felt it aggravated the lower abs and iliopsoas. Is there certain exercises that should be avoided? I noticed that when I would lay down and lift the leg of the affected side, I would feel pain in that groin/inguinal tendon area along with in my ilopsoas.

I had a similar problem. It felt like my hip flexor and quad kept getting strained but really I think the source is the hip? I got rid of that "strained feeling" and I am running relatively pain free, but not my lower abs feel weak and strained. Has anyone had a similar experience?

hip fai and sports hernias can and often do go together.

but, hip fai can also cause inguinal pain.

for hip fai, see a doc that specializes in hips and get a supine, fog-legged xray. that in additon to exam can dx hip fai.

sports hernias- well, a tougher dx but you can do searches on this site to come up with much info.

I was told by a doctor that it was not a sports hernia. I also had an mri on my groin and hip. it was not an adductor or labral tear. Is it possible that it is inguinal tendonitis or maybe something with the iliopsoas?

how do you go about that?

what doc checked you for a sports hernia?

someone in New England. I saw an ART therapist and he said it was possibly by rectus abdominius. I am keeping on runnning, he just said stay away from leg lift ab work stuff for a bit.

i saw 34 docs over 18 months before my proper diagnosis of bilateral sports hernias

then a year later i got dx'd with the hip fai cuz i got fed up with albany and hudson valley docs and headed down to nyc.

Its possible that your hip flexors are now very very tight and need to be released. I had this too and found out later that my internal rotators and rectus femoris were extremely tight due to too much rotational torque while running on track curves. Like u, I could not lift my affected leg w/o pain in the groin, until I got my entire hip joint musculature stretched out.

Its possible also that u might be over-reliant on that leg/hip for standing or for support when u run. If its a shorter leg then this could create the same issues.

But I agree w some of the others that if u r unsure and nothing works, better to see the doc if u can afford it. I shd also say that try some foam rolling/massage and stretching yr hip flexors and rotators b4 seeing the doc, and strengthen your glutes. Strong and functional glutes go a long long way in helping w hip problems, unless u are already having a damn serious tear in that area right now, in wh case medical intervention will take precedence.

Along w all that watch yr posture and techq, esp when u run. If u tend to extend yr leg out front u make the hip take way too much of the impact and torque. This usually comes tog w a trunk that's way too erect and not relaxed. There has to be a general forward total body lean fr the ankles, so that yr glutes can fire to take yr hip backwds w each stride. This body position also enables yr foot to land under yr hips and not in front. In this position, the whole body takes the landing shock, not any isolated area. This means that yr body shd move first, then followed by yr legs.

If yr leg extends forward to 'grab' more ground w each step, then the problems start trickling in, first fr the feet, then the knees, or for others its the low back and hip first, bec this running style makes yr leg a pivot over wh yr upper body leverages itself to 'vault' over the ground. The hip joint area will then receive most of the pressure. It then doesn't matter how strong u r in the hip muscles or the postural muscles; wrong movement will result in injury in even the most well protected of joints.

Hi guys, thanks for all the information. Its so relieving to hear other peole with similar problems. I am a physio and do triathlons. I recently upped my milage and did a 90lm trail run in three days and straight after and during had severe pain. I was treated by a highly qualified physio and was almost pain free, just night pain. I then decided to go for a mri with contrast to make sure everything was ok before I started training again but was sad to find out I had a labrum tear and fai on my left hip and fai on my right. I only feel pain on my left but my right hip clicks. I've never been able to swim breastroke and tried gymnastics when I was small and couldnt do the splits... I now know why!!! I am going for a labrum repair and cam shaving on 18th aug. I would like to hear how you guys are doing now a year later... can you run at the same level? Any tips?? it sounds like iliospoas starins are common. should i avoid ab work? and swimming? cycling?

Hey there ... just read your post. I'm looking at getting an MRI soon to determine if I have FAI ... the symptoms (and my chiro) seems to be pointing in that direction sadly. Have you been "issue free" since the surgery. I'm an amateur racer, but more importantly want to be able to live without the pain! Who was your surgeon? In what part of the country do you live? I'm in Cincinnati and looking for an experienced ortho-surgeon with FAI.
