Topical lacquer applied daily (ciclopirox, or Penlac) has about a 30% success rate. Need to apply for about a year.

Oral itraconazole 200 mg daily for 3 months has about a 60% success rate. Can be rough on the liver, so drinking alcohol not advised. In either case, you can get it again. If it's not bothering you much, perfectly fine to leave it alone.

Both are prescription only. Over the counter creams/liquids generally do nothing.
