Many Primary Care Sports Med doctors don't necessarily know as much as you would hope they would regarding physiology or even biomechanics. Many learn things by the books alone, and do not understand what is involved in elite level running. There are plenty of resources that list one of the treatments for overuse injury is simply reducing training volume or intensity, and make no mention of what impact this will have on performance.

I have also dealt with physiologists that question why somebody has to run 45 miles a week to prepare for a race that is only 5K long. Many people just don't understand...
