Claim was denied Body: My health insurance plan started in May 2011 and everything was fine in 2011. I received infertility treatments and they were covered. At the end of the year, my insurance informed us that there will be some changes in 2012 and they sent a new Benefits Booklet. I sent an email asking if my infertility benefits will stay the same.
