I may just be retarded, but I once tried running on my treadmill at a decent cooldown pace (like 6:30 or so) with the lights off because it gets very hot in my basement. My treadmill backs up to a wall. About 10 seconds after I started the treadmill, the treadmill belt slipped and I went flying backward and got my foot stuck between the wall and the treadmill. It completely screwed up my Achilles tendon. I had to bike and swim for two weeks, then run really low mileage, and it took about two months for it to stop hurting on runs.

I tripped and fell on a treadmill at an 8:30 pace but never when I cranked it to 5 minute pace. Ended up scratching my shin and bleeding and flew backwards.
Wasnt too bad but if my uncoordinated ass was going to fall at 830 didn't care about 5.
No because the correct response is to do some push ups after a fall and give the nearest person of your sexual preference a "how you doin?" look.
I always attach safety cord to my shorts if I'm going to run 10K pace (~5:45) or faster on treadmill. Luckily no accidents so far and I run on treadmill a lot during winter. Sometimes I would accidentally pull the cord and it would stop and I don't fly anywhere.
During recovery runs I frequently hit a plastic bit that's on the front of my treadmill where the belt comes out. It's because I'm going ever so slightly faster than the treadmill so I do a half second stumble but I never actually fall or have to use the hand rails.
