I've been running in Hokas since they introduced the Bondi - don't use heel lifts per se, but do remove their cushioned liner for a flat one with my orthotics layered on top. Those have a bit of a heel "lift" in that they do have some structure in the heel - not sure if that's what you're looking for? It works great for me. Missing a ton of meniscus since for over 10 years, so Bondi's really changed my ability to run pain free (as an old slow masters runner).
Thinking more of a wedge to put under the insole. I have custom orthotics as well, but I don’t believe they add any lift.
I’ve been doing this for the last 6 months with all my running shoes, including Hokas. I think it has helped in managing my Achilles tendinopathy. I use these 0.3 inch Spenco heel cushions under my insoles. They’re not a wedge per se, but they stay in place and don’t feel like there’s an abrupt drop off between the heel and midfoot.
