In New York you normally register with a driver's license, non-driver ID or social security number and, in the case that you are changing polling districts, provide information about where you previously lived. The driver's license/ID database normally has a social security number in the record for that person. These databases can be queried across states and it seems pretty safe to say that is done so in this case.
When you change driver's license address (within NY state) your polling district is changed as well.
