I have had a long lasting (9 months now) hamstring injury. it was an insidious onset, almost certainly not an acute injury like a tear. The injury has evolved a bit over time, but for a while now, it could best be described as a sensation of weakness while running (no noticeable hitch in my stride though), with a significant amount of soreness and tightness afterward. The progression of the injury that makes the most sense to me is that I developed tendinosis in the high hamstring, which was resolved through the conservative treatment modalities, but at some point the sciatic nerve became entrapped by fibrosis.
I have tried many of the conservative treatment modalities with no lasting changes (graston, ART, dry needling, along with numerous rehab exercises including nerve flossing). Nerve entrapment makes a lot of sense to me as I would expect any muscular injuries to have healed long ago.
I found some ncbi articles that talk about how hamstring injuries can heal, but sciatic nerve entrapment persists. This diagnosis makes the most sense to me. There is a procedure called US guided EPI, which basically uses a galvanized needle and a current to break up the fibrosis entrapping the nerve. But unfortunately, this seems to be a procedure only available in Europe. The closest alternative I have in the US is hydro dissection.
So my question is, has anyone had any experience with this type of injury, US guided EPI, or nerve hydro dissection?
