Anyone dealt with middle toe pain, it hurts right in the toe, kind of on one of its joints (not by ball of the foot), it hurts as I push off the ground. Any ideas as to ehat I have? a stress reaction, arthritis, etc.? I am a HS runner by the way, I took a week off due to ilness, it stopped hurting, then hurt again today.
It's not arthritis... only way to tell would be by going to doctor's office. Stress fractures in the foot usually occur in the metatarsals, not in the phalanges (toe bones); so, I wouldn't freak out yet, but go see a doctor if is hurting. Proper imaging is the only way to be sure.
that it stopped hurting when you took a week off is a big clue.
the thing to think about is if you recently increased your mileage or intensity, in shoes that are not wide enough for your foot. at low training volumes shoe width, especially in the toe box, is not an issue, but as you increase the training load correct width assumes greater importance. if you have another pair of running shoes try them for a few days. if that doesn't fix it, go to a proper running shoe shop where they do gait analysis, tell the assistant about your problem and make sure they fit you with the correct width of shoe.

Just an update. I actually had switched from mostly running in altras to running in adidas, big toe box change. I have been running in some older altras for two days and I've been getting much better, so thank you. Hope it gets totally better in the next few days.
I have this exact same issue. Middle toe on the left side, only when pushing off of it. Been to 2 podiatrists, 2 sports doctors, and 1 chiropractor. Wearing Hoka Clayton 2 and Feetures AND working on stride/cadence has helped but not cured.
What shoes did you switch to? I've had this issue for about 4 years and have only learned to deal with it.
