How would you guys train for a road marathon with about 1600ft of elevation gain?
How different to training for a flat marathon? Lots of hill reps/strength work or more focus on aerobic miles?

I would train with lots of hilly long runs - you need to get strong, not fast.
For my Big Sur (very hilly course) I put more emphasis on hill repeats, and long runs with hills involved. For a marathon with few hills I would've focused on pure mileage/endurance. I would say try to do air-squats in your free time as well, to strengthen those hill-running muscles like your glutes and quads.
Am I living in the twilight zone? The Boston Marathon weather was terrible!
