Obsessive-compulsive thinking about running.
Anything fatal. Trauma from a motor vehicle accident is the first thing that comes to mind.
Decapitation.

I've had all of the aforementioned injuries listed except stress fractures, but even then I'd say PF is the worst because I know people who've literally had it for over a year. At least with a stress fracture you quit running, it heals, then you start running again. PF has no rhyme or reason, which makes it scary.
Any sort of high hamstring problems. Like PF but even harder to resolve.
The one I have right now.
I had PF for a while—and I would take that over anything else. The only other thing I've had was a little tendinitis below my knee, and that sucked. PF just hurts a bit, and sometimes part of your feet hurt REALLY bad after or during runs. But its totally manageable pain and usually goes away randomly.
I remember a previous post about this and someone mentioned they had a stress fracture in their leg (or wherever) and an infection developed and they almost died
I had a femoral stress fracture that fully separated one day. That kind of hurt. A lot.
Yeah, I hate when that happens!

Did it go kinda like this?
http://runningmagazine.ca/kenyan-runner-takes-horrific-controversial-fall-in-toronto-breaks-femur/I hate when I go running and then get shot. Worst running injury.
I think we can agree that the word "injury" should be the common or dictionary meaning; so that when people say they have a running injured it really does not mean death/dying.
In my mind the worst injury would be the one that prevents running and racing, and takes the longest time to recover/heal where you can't run or race. Although, some people might argue the one that causes the most pain is more serious.
My vote is for tibial stress syndrome just because it has been for me an endless problem stopping me from running for weeks at a time for years, to the point that after a bad season and a redshirt year this year, I may pack it in next year if some miracle doesn't cure my shin problems.

I'm sure Chris Solinsky was pretty upset when he tripped over his dog and ripped his leg out of his hip.
I had a friend who had a torn Achilles ran on it and it ripped it sucked deep into his calf and left his foot paralyzed as he layed there on the ground and nearly died. HIs calf on his right leg remains like 5 inches smaller than the other leg and he will never be able to run again. He said they had to clamp his calf and pull his achilles out and re-attach it. How horrible is that?

I can relate to that nuisance
There aren't many injuries more painful or debilitating than back injuries. I'll go with that.
Anterior compartment syndrome and it's not even close
