On the first metatarsal? If so, you probably have a bone spur. If it is on a phalange, it could be a fracture. Bone spurs are usually on the top of the first metatarsal below the joint.
If it is a bone spur, you really need to get an x-ray to see how bad it is and what sort of damage there may be to the joint. The options for a bone spur are not good. Surgery is risky. The joint does not always recover well after the surgery.
I was able to avoid surgery by resting it until all the inflammation went down (ice does not make much of a difference, just give it time). Then, I put a "cluffy wedge" (google it) in my shoe. I actually put it in backwards so that the wedge supported the first metatarsal. After a few months, the pain went away and the bone spur stopped progressing. Range of motion is still limited, but I can run on it without any problems.
