How many people in the 800 are going through this? Is it actually enough to call it "impacting". The only person that I know of is Semenya and maybe one other runner.

If this is indeed true, I suspect the cause is that is an event where both pure speed and aerobic ability are required. Men are better on average than women with respect to both of these attributes. To be good at the shorter or longer events, women only need to be elite at one of those two aspects, while to be good at the 800 they need to be elite at both. Easier to find a man that is above average at both than a woman who is elite at both.

A guess, but probably has something to do with the countries where the condition is more prevalent and where they specialize in distances
Dutee Chand runs the 100m, and India is not known for sprinters. Castor Semenya (800m South Africa), Shanti Sounderajan (800m India), Margaret Wambui (800m Kenyan). I was with the OP that says the 800 is over-represented, though it may be a small sample size.

Every high school in the country has a boy under 2 in the 800. They don't have a sub-11 100m runner or sub 15 5Ker though

In sprints it is a moot point because of all the steroids which go hyperer-andro. They are wayyyyyy bigger and stronger than the 800ers.
In distance it is moot because male distance runners are all girly men anyhow.
