Not sure how it will start, but it should end:
Sincerely,

I love you and love being coached by you. But if you don’t want to coach me anymore, please tell me now. I cannot go through another year by myself like this past year. If you don’t want to coach me, please tell me now so that I can figure out what I am going to do. I can’t stress enough how much I love you coaching me, but I can’t handle these types of threats leading into an Olympic year.
Even though we may disagree on some things, I think we'd both agree that
we care a lot for one another. I hope that never changes. I have loved you for years now and don't see that changing.

You should introduce yourself (name, school, grade, city, PRs). You should let them know why you're contacting them (i.e., I'm interested in coming to your school next year). You should let them know what you would like them to do (are you trying to come by for an informal visit, do you want more information, do you want to be on their mailing list, etc...). You might also say a few things about yourself.
I'd avoid making the e-mail too long. I'd also avoid asking a lot of questions. Focus on the important things. You can work on the details later.
