Plenty of athletes get hurt but when is the coach to blame? Are they having their athletes train too hard with enough rest? lots of coaches have different runners with the same type of injuries year after year. Is that their fault???

Really depends. It may not be "overtraining" but "under-recovering". For example, if the athlete isn't taking care of themselves away from the coach (lack of sleep, drinking too much, etc.). I've also seen collegiate athletes who do everything right away from practice but have horrible luck and their bodies can't hold up.
imbalances and not enough recovery.
bad coaching if injuries due to not enough recovery but the athlete should be able to communicate to the coach how he/she is feeling instead of blindly following so as to avoid overtraining.
