Is there some poor guy who ran like 2:14 outside of the window and then got hurt and was never able to do it again? I think something like that would be much worse than coming up just a few seconds short inside of the window.

most likely some fit guy in 1980 that had Carter boycott the Games b/c the Russians invaded Afghanistan. Then the guy had to get a real job
Jimmy Hearld ran 2:12 but got hurt in lead up to the trials in Birmingham and never ran another marathon if I am not mistaken.
Not quite what you asked b/c he did qualify for the trails but just did not get to run in them.

Assuming you mean just the Marathon Trials I think it's Craig Virgin 2:10:26. However he did win the 10,000m at the 1980 track trials and was 2nd in 1976 and 1984.

Yeah, the answer to this depends on exactly what you mean by "making" an Olympic Trial. Eamon O'Reilly had the fastest time in the US in 1968, 2:16 plus but did not run in the Alamosa Trial race. In 1970 he ran 2:11 plus at Boston but did not run in the '72 Trial.
