Why would you be obese? You are obese when you take in more calories than your body can use.
To be honest, and I see a lot of this on this board, but I suspect you have an eating disorder, since your question is somewhat irrational. Most people can go through life just eating when they're hungry and at most, watching the fat content, etc. You seem to be unsure how to even determine if you are hungry for fear of being fat.
You may want to seek professional help. It's nothing to be ashamed of.
Actually, most Americans do not do anything at all like you describe. In the book "Eat to Live", Dr. Joel Fuhrman talks about how most Americans mistake cravings for hunger and don't know what true hunger actually feels like. (and he didn't mean that as they don't know what it means to go hungry, or starve.)
