there are also speciality journals that are considered top tier. cancer, neurology, obgyn, orthopedics etc all have journals that are considered top tier.
Also The Lancet.
But as mentioned nearly every discipline in medicine has a top tier journal or two for its field.

Based on impact factor: NEJM > Lancet > JAMA > Br Med J > Ann Int Med
I would give you a hierarchical list if this was before the year 2010. Since then medical research journals have been usurped by thousands of internet publishers of medical research from across the globe. The internet is the best place to publish material for peer academic and clinical review. Thanks for your interest.
