It bothers me more when people say it in the middle of sentences, and then keep going, and then say it again.
I had a very brilliant and beautiful girlfriend from Texas whose one vocal peculiarity was how frequently she asked "do you understand what I'm saying?" I always understood what she was saying. She was insecure and had no reason to be. I tried to help her with that.
People don't talk like that where I'm from. Sunburns are common around here. Nawm sayin?
It's a "Valley Girl" expression just like "you know". Nothing more, nothing less. College kids have picked it up the last few years. Next year there will be a new fav. Just sayin..........
I feel like I've posted this here before, but....
it means they are too stupid to properly construct a sentence and if you continue talking to them you will become just as stupid

Either way, they are unconsciously aware that they are painfully difficult to listen to & compensate for this by littering their muttered utterances with some variation of "do you know what I mean?"
Another possibility is that the speaker is mentally diminished ("tard"), and when attempting to listen to others, frequently becomes confused & doesn't know what the other person is saying. Perhaps a highly literate and intelligent person is dropping down to your level of neanderthalic stupidity and - in an attempt to communicate - is employing common idioms. If you hear these sort of phrases frequently, it may be more of a sign of your own stupidity than theirs.

The use of the term is something that annoys me but is mostly harmless.
Are far bigger problem and sometimes drives me to near homicidal thoughts, is when a supposedly educated person uses "at the end of the day" while speaking.
Please tell me what the f*** that means.
I hear "you know what I mean" as much as "you know what i'm sayin". Annoys the hell out of me. Along with people saying "like" 100 times in a story they tell you.
It means they don't really know what the hell they are saying so they hope you at least do.
